Are you by chance listing the URLs on separate lines using a "\" for line
continuation? Something like...

  start_url = url1 \
                     url2 \
                     url3

I had a problem similar to what you describe at one point. Only a subset of
my URLs were being processed. While trying to figure out what was going
on, I noticed that there was a space after one of the "\"'s; on the line
containing
the last processed URL. When I removed the space, the next run of htdig
seemed
to work fine. I never took the time to verify that the extra space was really
the
problem, but I know a number of other UNIXy things will choke if there is
anything
other than a newline following the "\".
