But it doesn't modify the MIME headers to reflect what it
did, does it?

Seems wrong to me.  Likewise the inverse transformation
outgoing.

I know by experience that some MTAs convert 8-bit<->Q-P,
modify the Content-Transfer-Encoding:, and even add a
X-Comment: line acknowledging the modification.
