I find it easier to never have duplicate volume serial numbers. It requires a 
little work up front, but the required OS support was available in OS/390, so 
it's hardly leading edge.

Yes, that's easy. Simply provide new device addresses in the LOAD panel.

Yes, it is possible, however it most cases not needed at all.
Method: Activate the LPAR, but do *not* start (IPL) the system. Then use
SE/HMC features to make the channels offline to that LPAR. Then IPL
(Load) the system.

It is not needed, because you have unique device number in the IODF. And
the numbers (aka addresses) are used to point SYSRES volume and IODF
volume.
Depending on some factors you could have duplicate volsers. In that case
during IPL you will get (a lot of) WTORs asking which address to choose
offline. However properly prepared OS Config will prevent it by making
some devices as offline. Or not defined.
