As Michael pointed out, you need a user associated to the database (or two 
actually).
That user should have a password, make sure the user/pass is consistent between
what MySQL has setup and what Icinga is using.

It's simpler than you think. No need to start over, just get it consistent.
If you created the user, assign it a password and get make sure your cfg
file shows the same.
