Signing on each attempt is fairly common, at least among my peers. I think
this might just be because certain popular commercial MTA products do it
that way by default, and in practice it doesn't usually add enough load for
users to need to change the signing behavior.

In any case, it is an important distinction to make, and changes the
equation quite a bit for appropriate values of x=.
