Did you set the alternate SMTP port to 25 and force AUTH?  I found that the alternate SMTP port with forced AUTH will not honor allowed IP's, it will only allow true AUTH.  This is reaching a bit, but it's the type of thing that someone might accidentally set. I think that if you were able to identify the exact conditions where things worked and where it didn't work, it would help identify the issue.  For instance, if it always works for some and always doesn't work for others, or if it randomly will work and not work for a particular computer that is trying to connect.
