outside/public ip of NAT box to reach the inside/private ip of Exchange.

ok, this "additional" NAT configuration item of port redirection is needed,
we do this, too.
Sheldon, if you can manage Dan's port redirection trick, then you can drop
the Exchange ETRN and Imail relay rigamorole and keep their SMTP traffic
out of Imail completely. Their Exchange would be able to play directly on
Internet like a big boy.
But a fixed ip at their end of the DSL line for the NAT box's outside i/f
is still required.
