Frank,
I too have been using and supporting Imail since version 4 and in almost
every instance that emails get stuck in queue DNS has had an issue. My only
thing is that being stuck for days can also mean that the queue timer is
either set too high or the amount of retries is above 20 or so or smtp is
hanging on a bad email or corrupted domain setup.
I would go to another name server using nslookup and try doing lookups
and reverse lookups back to the domains in question to be sure that reverse
is working correctly. I do understand that some of the mails are going
straight back to the same box so you may want to make sure that localhost
has 127.0.0.1 in it. the site I use for testing DNS is:

I've torn out what was left of my hair over a particular instance just
like this only to discover that I had something mis-typed or in one case on
a different class "C" reverse delegation was wrong.
My company just spent 2 weeks at a local college re-doing their network
and just before we finished they mentioned that they were using Imail and
could not get mails to a certain site occasionally. We did lookups and
discovered that the site in question had an old IP address in cache on the
destination DNS server. We asked them to flush it and reboot DNS (bind 8)
and viola emails starting leaving the queue while we watched. We also found
the timers and retries were very high. We did set them to a more reasonable
amount and sent emails to invalid addresses and they now were getting
returned in about 2 hours versus days
