You can set Exchange to automatically contact your mail server and issue the ETRN command (provided you have 5.0 Service Pack 2 or later). It works quite well.
 
Just look in the Internet Mail Connecter in Exchange. there is a "Use ETRN" option under "Dial-up Connections".

you need to point out the big gotcha that stops a lot of ETRN wannabees:

the Exchange host site must have a fixed ip address to issue ETRN, the ip address being the only security ETRN offers.  Because the Imail SMTP Client opens new smtp session to that ip address in NT hosts to unqueue that domain's mail.

The dump-it-all-in-one-box-and-Exhange-POP-it-out uses POP3 security so a fixed ip is not necessary.
