Hijacking is a specific tactic where a spammer relays his spam through your
server to his targets on other servers, so it looks like your server is
doing the spamming, and you get the counter-attack and blackholed, while
the spammer escapes. This hijacking can occur
1. you have no anti-spam defenses at all. Many 1000's of mail servers are
like this. They are mostly listed in RBL and/or MAPS RSS databases.
2. you restrict relaying to senders whose "MAIL FROM: [EMAIL PROTECTED]" is
a local domain.com on your mail server. The spammer forges the
[EMAIL PROTECTED] header and relays his spam.
3. you restrict relaying to ip's that you "trust" but do not stop ip
spoofing at your gateway router, so the spammer spoofs his sending ip
address, your anti-spam defenses are fooled again and the spammer can relat
through your server.
The last way isn't really hijacking, but a valid user with either a
authenticated mail account and/or sending from a local ip address, just
spams from your server. This is indefensible technically with Imail. You
have to fix it by detection and then cancelling the mail account as in
violation of your Terms of Service.

In the upper half of the commercial products, I'd guess. The most used
mail server is free: sendmail, and there many other free open source SMTP
servers, POP3 servers, IMAP servers.
