I've seen this when a user sent mail from Outlook with the mail format
set to Outlook Rich Text Format.  Anyone with Outlook got it fine, but
any other mail client (even Outlook Express) couldn't see the
attachment.  I changed the setting to Plain Text and haven't had any
more problems.
