There is a known race condition endemic to SMTP conversations in which prematurely broken connections between mail servers (due to hardware, driver, MTU or related packet-level issues) can cause messages to be resent. A similar race condition exists with POP3 over slow or shoddy connections. A more common cause is broken MUAs like Outlook which have error-prone methods of recording which messages (UIDLs) they've already seen.
