By examining the file, you can pretty easily find out how IPs are 
added.  The main problem, though, is that the file is undocumented, and 
there are a number of fields related to the whole file that aren't easily 
interpreted (and could potentially cause problems).  One person on this 
list did write a program to alter the file, but whether you use that or 
write your own, it's a "use at your own risk" situation.
