Can I ask how you are able to reject E-mail with .pif files? Do you reject after the data is sent after the DATA command (which lets you see there is a .pdf file, but requires all the bandwidth to be used)?
All attachment rules can only be applied only after the DATA command is complete. The 5xx/2xx response code to the DATA command is when the decision is made.
