This works pretty good too just change example.com to your site:
<div align="center"><center>
<table border="2" width="640" bgcolor="#FFFFCC" cellpadding="2"
cellspacing="5" bordercolordark="#000000" bordercolorlight="#CC9966">
<tr>
<td width="145" align="center" bgcolor="#FFFFFF"><p style="MARGIN: 0px;
LINE-HEIGHT: 100%"
align="left"> </p>
<div align="center"><center><table border="0" width="145" height="83"
cellspacing="0"
cellpadding="0">
<tr>
<td valign="middle" width="158" height="77"></td>
<td valign="top" width="224" bgcolor="#FFFFFF" height="127"><font
face="verdana,arial,helvetica" size="-1"><b><form
action="http://www.example.com:8383/login.cgi" method="post">
<input type="hidden" name="page" value="login"><input
type="hidden" name="Use_Cookie"
value><div align="center"><center><p>User ID<br>
<input size="15" name="userid"><br>
Password<br>
<input type="password" size="15" name="passwd"><br>
<input type="submit" value="Sign In" style="font-family: Tahoma">
</b></font></p>
</center></div>
</form>
</td>
</tr>
</table>
</center></div></td>
<td width="521"> </td>
</tr>
</table>
</center></div>
If you put mail.example.com www.example.com into the host aliases then
The client can just enter there user name (without the @exampl.com) and
There password.
