I'm mind-boggling new to this list, and a quick search of the archives returned too many false-positives to be useful.

Either (a) valid SMTP AUTH credentials passed using an encryption method supported by the server--type EHLO to see a list of AUTH methods supported--or (b) a source IP in the 'Relay for Addresses' list, if 'Relay for Addresses' is in use.
This has been gone over a mind-boggling amount of times on this list.
