We have a customer who just started experiencing this same problem and I have been unable to find a cause for it yet. It is only happening with one virtual domain on a server that hosts over 1000 virtual domains so I'm guessing it may have something to do with the particular MUA that this client is using but I haven't been able to contact the customer and ask that they try with a different one yet.
