How's the disk space on the spool drive? I've seen blank e-mails being
sent when the free space gets lower than 5-10% of the total drive space.

It sounds like the D*.SMD file is getting deleted.
Are you running any AV software on the server that might be doing
that? If, for example, they are running a broken mail client, all
their E-mail might contain vulnerabilities, in which case all their
D*.SMD files could get deleted. This could happen if you are
running an on-access virus scanner that scans the spool directory
(it won't happen, of course, with a mailserver virus scanner).
