Sounds like to me that you are pulling the first description that comes out of 
the dictionary and not the one for a particular locale. I can't see why else 
these other concept_names would be appearing.... check to make sure that the 
concept_name query is pulling the preferred for language and locale are set....

I don't use HTML form entry, so can't be sure why....
