I am building an HTML form that a physician will fill out when referring a
patient for additional care. One of the options on this form will be
"referral to" which needs to be a fixed list of hospitals and other
specialists.

I'm trying to decide if I should have a concept such as "refer to" with
coded answers (probably all of which would be contained in their own class)
or if I should have this concept be a text datatype then use html forms
style=location.  One downside to this is that I don't really want users to
be able to select various hospitals and physician offices as the location
of other encounters.

What do you think makes the most sense based on everyones' experience and
the data model?
