I am still not exactly sure I understand what you are trying to do... Do you 
want a dropdown list of potential classes of referral locations (hospital, 
clinic, district hospital, etc.) or specific locations (Hospital A, Hospital B, 
etc.)? We actually thought that it would be a good idea to be able to use the 
same location function that draws coded locations for the encounter, and be 
able to use these for this field. However, as you suggest, there should be 
flags on the location table which restrict which appear for which context of 
use. For xforms, it was possible to remove inappropriate entries from the 
dropdown at the time the form was deployed. I presume the same would apply to 
HTML forms if you loaded the locations into the form.

In the interim, we just used a text field and entered a numeric code for the 
facility...
