Ok now I am even more confused.
I noticed that the upgrade_log file I was looking at was about 6 months old.
Created, I assume, when I upgraded to b65.
This may mean that the problem has nothing what so ever to do with audiohd.
I have removed /var/sadm/system/logs/upgrade_log* and tried the upgrade again.
Here is what I saw happened when I tried to upgrade from b65 to b78.
* Picked US-English for the keyboard layout.
* Got to the GUI installer with no problems, selected English.
* Chose "Upgrade" not "Install".
* It spent ten minutes "checking for sufficient free space . . ." with the
head of the DVD drive going crazy seeking all over the place on my laptop.
* I accepted the license terms and pressed the upgrade button.
* The progress bar appeared.
- It quickly got to the 7% mark while "Preforming upgrade check" it stayed
there for 10 minutes while the DVD drive went crazy again.
* The progress bar vanished and it reported:
Upgrade Failed
Solaris upgrade did not complete normally. The system has been
restored to its previous state.

When I clicked on "Solaris upgrade log" a window popped up.
The window was empty.
How can I get the installer to tell me the reason why
the "Solaris upgrade did not complete normally"?
