Thank you for your suggestions.  Unfortunately, the live cd also failed to 
boot.  It never progressed past the same "Probing for device nodes ..." message.

When booting with the grub options you suggested the system hangs after 
printing some 20 progress dots to the console.  If, however, I use the grub 
boot options you suggested *and* include the options in single quotes, I see 
more logging until the "Probing for ..." message, but the system still doesn't 
progress beyond that.  I imagine this is a fluke and that surrounding grub 
options with quotes isn't what you suggested, but I figured I'd give it a shot. 
 I waited > 20 minutes and the debugger never starts.  The new logging output 
that I can see is:

I'm willing to hook up a console capture device if necessary to get more 
information.  What do you suggest I try next to isolate the problem?  My next 
move would be to swap out hardware (perhaps starting with the LSI HBA) and see 
if I have more success.  But this seems like too much voodoo to count on.
