We've worked out the procedure below to fix the icons. Please try it out.
Thanks.

Getting Up-to-date Icons When Upgrading InterMapper 4.5 from Beta to Release
Document Actions

We have adjusted the default icons several times over the course of the beta
test of 4.5. This document describes how you can make sure that you have the
latest version of these icons.

In order to ensure that you are using the latest version of the default icons,
please take the following steps directly before upgrading, or at any time after
upgrading. This applies only to testers who installed versions earlier than
4.5b4 and have not done a clean install (including removing the InterMapper
Settings folder) since.

        1. Stop InterMapper Server/Daemon. This may be done from the
InterMapper
Control Center on Windows or Mac OS X. For Linux, use sudo
/etc/init.d/intermapperd stop.

        2. In any running copy of the InterMapper GUI or InterMapper
RemoteAccess, open
the debug window by choosing Help->Diagnostics->Client log... Choose Misc->Empty
icon cache

        3. Close/quit the InterMapper GUI or InterMapper RemoteAccess from
step 2
completely. (Note that you may need to perform steps 2 and 3 on any copies of
the InterMapper GUI or InterMapper RemoteAccess on any machines that have
connected to a test version of InterMapper 4.5 older than 4.5b4, whether or not
they are currently running.)

        4. On your InterMapper server machine, open the InterMapper
Settings folder. Go
into the Custom Icons folder and remove the Default.zip file.

        5. You may also wish to remove the Default folder, also in the
Custom Icons
folder. It is not necessary to do so; however, there may be a few icons which
have been renamed; both the old and new version icons will be in the Default
folder after following these steps if you do not. The old versions may not
perform as expected. On the other hand, if you do remove the Default folder and
you have used the older version of the icons, you will see the question mark
icon on your maps.

        6. If you are upgrading, do so now. If you have already upgraded,
you may start
the InterMapper Server/Daemon again now. This may be done from the InterMapper
Control Center on Windows or Mac OS X. For Linux, use sudo
/etc/init.d/intermapperd start.
