Review comment:
       This section is about retrieving the a potential title in the document. 
If the first meaningful (non-comment) node is a *Heading*, then it is 
considered as the title. Here, we're skipping comment nodes to check the first 
non-comment node.
   
   Comment nodes are preserved in the document that is produced.
