For now let's hope that you were the victim of a unique bad luck particular circumstances, whatever - bad things happen. But of curse it might be that there is a real OOo bug. If you have time and nerves (and the security of successfull fallback to 4.0.0 it would be great if you could do a second attempt after you checked your installation file with md5 checksum and so on ... If there is any small chance to get the problem reproducible for a developer, of course we will follow up your report - especially if we get similar reports.
