Summary: Move OM request handler fault injection into a test

{\{OzoneManagerRequestHandler}} has a test-only fault injector that runs on
every write request, and \{{OzoneManagerStateMachine#getHandler()}} casts the
handler to the implementation class to reach it. Move fault injection into a
test-only \{{RequestHandler}} decorator and remove the cast, so the handler can
be a tracing proxy. No functional change.
