Recently a plug-in was added that allows you to do this; See http://cvs.sourceforge.net/viewcvs.py/itext/src/com/lowagie/tools/plugins/ and look for XML2Bookmarks and Bookmarks2XML. Of course you can't extract such an XML file from a PDF document that has no structure. You will have to find a way to construct this XML.
