Is the width value for the bullet available in the PFM file?

That's a good question :-) I haven't been able to find a spec for the PFM
format.

I used the pf2afm utility in ghostscript to create a .afm from the .pfm.
This had an entry for bullet in the metrics list. I think the problem was
that the encoding was set as font-specific - this agrees with the behaviour
I saw when I debugged iText.

I dug out the original .afm for the font and noted that the encoding was
Adobe standard. When I changed my program to use the .afm instead of the
.pfm the bullet width was non-zero.

My problem is that I may have to deal with Type 1 fonts installed on other
people's systems. In many cases all I'll have is the .pfm and .pfb in
Windows\Fonts left by the MS Type 1 installer. Do I have to tell people that
they need the original .afm files?
