This is one of those things where the PDF reference says one thing and
Acrobat does another. The PDF reference says that the destinations
define the page by an indirect reference but apparently Acrobat also
accepts an int with the page number and already did that in Acrobat 4 at
least. I'll "fix" it in SimpleBookmark.
