It has to do with the RTF Specification and the way readers function. Some RTF Tags can be ignored if not recognized. There are generally 2 images in a Word doc for each image you see, but not always. I do not have an explanation of how/why/when this occurs though.
You may want to try embedding a different type of image. To understand it, you should review the RTF Specification.
