We have this tool called RUPS.

It's the Dutch word for Caterpillar, and it's an acronym for Reading and 
Updating PDF Syntax. It's not a finished product (you can't use it to 
Update any syntax yet), but as soon as the second iText book is 
released, there will be time to turn it into a finished product for 
developers.
For the moment, you can only download it from the SVN repository at 
SourceForge, and you have to build it from the sources yourself.
