the 100s of pages. Would that make a big difference to the most optimal

That's solved in the "Page X of Y" example in chapter 5.
You provide a PdfTemplate that will act as a placeholder.
As soon as you know the total number of pages, you add it to the
PdfTemplate.

You're now creating it in three passes (which is fine).
You could create it in one pass (which may not be necessary).
