That's what happens if you use getUnderContent().
As expected, it adds new content UNDER the existing content.

What makes you think there's such a thing as "topmost" UnderContent?
Are you saying the page content of your PDF doesn't consist of a single 
stream, but of an array of streams? In that case, your requirement is 
feasible. If not (and I'm pretty sure it's not), there's no way a 
program can find out what you mean by "topmost" UnderContent.

The best way to add a watermark, is to add it ON TOP OF the existing 
content, but to make it transparent.
