Using PdfCopy instead of PdfSmartCopy could be the culprit.
Although you shouldn't ever have a situation where file A + file B + 
file C results in a file size that is much higher than the sum of the 
file sizes of the three separate files.
I don't remember having seen such a situation, except in a distant past 
due to a bug in an old iText version. It's important to tell us which 
iTextSharp version you're using, maybe you're experiencing a bug that 
has been fixed a long time ago.
