3. Looking at the internal structure of the PDF document, I see that the document is Linearized. iText has never supported the creation of Linearized PDF.

I think you're using an unsupported fork of iText. Please don't expect the original iText creators to help you with a problem that may have been caused by obtaining an unofficial version of our software.
