Turns out part of my problem was caused by a dud config option for X that caused ivtvdev to default to to the first card. That mistake has been in there ever since I installed my PVR350 & ivtv drivers over 2 years ago. I've now been able to dual head using the 350s but the DMA errors remain & capture is impossible.
