Thanks for the suggestions. I've finally got my replacement card so I could
try it out. It behaves exactly as the old one (so there is probably nothing
wrong with either of them).

The above doesn't seem to affect anything, the signal is as bad as ever in
both cases.

A few observations that might be relevant:
The best image I've had so far, is at 670.62 MHz, where there is a very
noisy (but at least sync-locked) image from a station I can identify. This
station actually broadcasts on 663.250 MHz however. At 670.62, the sound is
perfect and noise free.

When using the S-video input, everything works perfectly. The problem seems
to be the tuner, and nothing else.

It's probably the same "cold boot" problem that many other people have, but
I can't verify that, since I haven't got windows. This is rather annoying
since I do have an I�C sniffer, and could fairly easily find out how the
windows driver is initialising the chips.

Yes, though I have already done that, of course. Can't hurt (much) to
give it another try :-)
