do you have bttv in /etc/modules?
If so replace it with ivtv.

If not who is loading bttv? Is that hotplug or something (dmesg won't
show, maybe syslog will)

The question is why is this loaded?

use modprobe instead of insmod

I got a PVR350 working on Debian. I followed Jarod's guide. except
that I've put the module related info in /etc/modprobe.d/ivtv instead
of /etc/modules.conf.d/ivtv

Check your modprobe related doc in /usr/share/doc and in the man modprobe.
