How odd, I'm still not sure why it's doing this, I guess it's using VBI insertion into the MPEG and our decoding is then trying to insert that into the decoder, I'm not sure this even works properly for NTSC (have never seen this work myself). I want to look closer at this, seems like something is not right in VBI reinsertion, although it did always have buggyness with ntsc from my experience, definitely something to look into.
