Hi!
There are 2 different cases to look at, when using sound with the TV-Out.

1st: The sound not relatet to Live-TV/Recordings.
This sound comes through your soundcard, as it also would do if you have
a normal Monitor connected to your VGA-card.
The image is coming through the TV-Out of the PVR350.

2nd: The sound relatet to LiveTV/watching recordings.
This the same as the dd-command you mentioned before.
In this case all playback is done by the PVR350. Video & sound are
coming out of the TV-Out of your PVR.

What i did to have both "types" of sound at once was:
Hook the audio of your PVR-350 into the line-in of your soundcard.
If you now watch tv with the PVR-output-option (Mythfrontend -
Configuration - TV - Playback - 5th Screen there. Enable the option "Use
PVR350-Mpeg-Decoding" and set correct parameters) the TV (and
recordings) are played smooth as with the dd-command, totally using the
mpeg-capabilities of your PVR350.
The sound is injected into your soundcard and immediately played back by
it. You just have to ensure you did not mute the line-in of your
soundcard and set correct volume-levels for it.
If you play sound generated by xmms/watching DivX or something else this
sound is played by your soundcard directly. In case of DivX
(non-mpeg-video in general) the image is decoded not by the PVR but by
your CPU and played back throug the ivtvdev-Driver created by John.
