Because many of the newer tuners are not available in older kernels, or even
very new kernels. The PVR cards seem to switch to different tuner models about
once a week :-)
I'm working on merging the non-ivtv modules into the v4l stuff, though.
And if you get recent stuff, then it is perfectly possible to use the kernel
modules only.
