As Chris says Adobe should recognize that you are using a screen reader
and will lead you through the set up for that.
If not, there is an option under the Document Menu to change
accessibility settings.

As for saving documents in text format...
under the File menu there is an option to "Save as Text."
I have found that when I first download a pdf file I am not able to save
it as text.  That is, the "Save as text" option is not available.
I first have to save it as a pdf file on my hard drive.  When I go back
and open that PDF file I can then save it as a txt file, and then it can
be saved as an MS Word document, or rtf,, as you wish.
