Larry I have been using a product called Text Cloner Pro for severayl years and it does very well for the price. It costs $100 and is designed for use with screen readers. You can get information on it at: http://www.readingmadeez.com/products/TextClonerPro.html I think you can still download a 30 day trial so you can try it before you buy.
