I agree some IE 7 pages can be a little unpredictable in this sense with
Jaws.   Try the following which will solve many instances of this sort of
problem. 

Press Insert F7   to  open the Links List dialogue box. Find the link you
want to download. 
Tab once to go to the  Move to link button and press enter.
Before touching any other keys ,now try pressing the application key or
shift F10 key. If there  is a save target link available it should show up.

This works   on most web pages but not absolutely  every page. The UK Seeing
Ear Library for example  is oddly unreliable using this method and I have
known it persistently download the wrong book.  You are better off using the
Jaws Cursor and performing a right click with either the real mouse or a
virtual right click using the * key on the mum pad. This resolves any
problems on Seeing Ear.
