What about Control+J to move to the download dialog, then whack enter on the file you want to start downloaded.

Try the left-mouse button, just to the right of the numlock key. If that doesn't work, I sometimes find that alt-n doesn't appear to work when, in reality, the problem is that JAWS isn't navigating as I'd expect. So then, after pressing alt-n, I try left and right arrow keys in lieu of tabs, and vice-versa. Sometimes the item you want appears in the system tray, but I doubt that would be the case here.

Does anyone have any alternatives when Alt+n doesn't change the focus to the
pop up window?  I struggle with this when downloading spreadsheets from my
bank account.  After pressing the download button I am prompted to press
Alt+n to "Open, Save or Cancel" and Alt + n doesn't change the focus to the
pop up window to select one of these options.
