Insert+a will only sometimes find an attachment. Another way is to read to the end of the message, route Jaws cursor to PC cursor, and then explore by hitting down-arrow to find an attachment or a list of attachments. You can then double click on the attachment with the left mouse button to bring it up or go into the File Menu and choose save attachments to save it somewhere.
