try putting jfw so that it won't cut out speech when in a dos session. 
Also, if you have a hardware synth, try loading your screenreader before
you load windows, so that the dos session might be able to be read.
I have not come across this situation myself so I really don't know what
the result would bring in doing this.  I use a product from computer
associates at www.vet.com.au it's a anti-virus program like mcafee and
norton's except that it doesn't cut out speech when a virus is found.
