in the first lost shuttle it had to do with something freezing which wasn't expected.

In this one, they knew somehting was worng with the left wing when it took off. It seems it was this same wing that casued it to explode on it's return.

There is very little in life that is a true accident. Most of the grief that befalls us is of our own making.Very sad as it is and not an excuse to judge people. no one is pefect.
