This looks great, but unfortunately I just can't get to seem to get this to
work with my existing form. I'm trying to implement this into my CakePHP app
using the below code:

When i switch this off, it works fine, but as soon as I enable this, it fails.
It only seems to fail on the actual upload, any idea what might be going wrong?
